What should I do to get ready for my first appointment?
We ask all patients to bring a few items with them on their first visit to our office.
- Please fill out our forms online, then print out the patient forms, fill them out, and bring them with you to your initial evaluation. If you cannot do so or do not have access to a printer/computer, please allow extra time when you arrive to complete these forms.
- Please arrive at least 15 minutes early to complete any outstanding paperwork so that you can get the most out of your evaluation time.
- Please dress comfortably so you can easily access your problem areas and move around.
- If you plan to use insurance, please bring it with you so we can make a copy of your card for our records.
- Please bring any referrals, prescriptions, or test results from your primary care physician, nurse practitioner, or physician assistant.
- Bring the contact names, phone numbers, and claim numbers of any case manager or insurance adjuster in case of an automobile accident or worker’s compensation claim.
When you call our office, please have your prescription and insurance card ready. This will allow us to confirm your insurance benefits and provide you with a coverage estimate.
